Supporting Carers


Patford House Partnership recognises the vital work unpaid carers do to alleviate pressure on the NHS and enhance the care we can provide to patients. With this in mind we put great emphasis in supporting our patients who are carers. A carer is someone who without payment, provides help and support to a partner, child, relative, friend or neighbour who could not manage without their help. This could be due to age, physical or mental illness, substance misuse, eating disorder or disability. 

If you are caring for someone or being cared by someone please make your GP or Reception aware of this fact so that we can update your records. We have patient consent forms which can be completed for patients and their carers to make things like getting blood test results and ordering repeat medication easier. 

When you are caring for someone you are able to request access to their medical records. Using SystmOnline or AirMid you can help them further buy ordering medications for them or viewing test results or sending messages on their behalf.

Do you have an emergency plan in place to ensure the person you care for is looked after in your absence?


It’s not something that we often like to think about – what might happen if I was in an accident or emergency – but it can help ease concerns to be prepared for such an event. In an emergency it may be that you are unable to communicate to an emergency responder or inform of them of important information. For example, you may be on specific medication and/or you might be a carer for someone, and this information could be key to pass on as soon as possible. The Carers Emergency Card is free and offers peace of mind and reassurance that things will be taken care of in case of accident or emergency. The card is available to all carers aged 18 or over including parent carers.

Carers Support Wiltshire

Carers Support Wiltshire is an independent charity established to provide support to Carers living in Wiltshire. Caring can be difficult and frustrating sometimes and Carers need to tell someone about these feelings. They aim to identify and help as many Carers as possible and employ specialist Carer Support Workers who provide confidential information and advice about caring matters and one to one support. Their free services include complementary therapies, social activities for some 'me' time, befriending services, counselling, education and much much more.
